After clicking on the Registration Form, you can fill it out in Word, save and then send back through the e-mail as an attachment ifixouches1@cox.net. Or you can print the Registration Form, fill it out and fax to 623-266-2366 (Please DO NOT fax past 9pm at night). You can also send the Registration Form by mail (Address located on form).

I do not charge the deposit unless you do not cancel within 48 hours of class, or you are a "no show", then I will deduct the $50.00

BLS deposit is $10.00

CTCA, and Trillium Specialty Hospital employee's=No cash due. We will bill your employer

We take Cash, Credit Card, Debit! NO CHECKS!

Discount given for CASH payment on day of class (ACLS=$15.00 off if paid in cash)
